Have these on hand — it takes about two minutes:
- Your insurance card, front and back — a phone photo is fine
- The policyholder's name and date of birth, if the plan isn't in your name
- Your claim number and adjuster or attorney contact, for an auto or work injury
A benefits check tells us what your plan says it will cover. It isn't a guarantee of payment — final coverage is determined by your insurer when the claim is processed.
