Tech neck is the pattern of neck pain, shoulder tension, and stiffness caused by hours of forward head posture at screens — and it responds well to acupuncture. Research shows that tilting your head forward multiplies the load on your cervical spine several times over, and that acupuncture produces meaningful, durable relief in chronic neck pain. This post explains the biomechanics, the Korean medicine perspective, what the evidence actually says, and what treatment looks like at our Koreatown clinic.
If you work at a desk in Los Angeles, your day probably looks like this: forty-five minutes gripping a steering wheel on the 101 or hunched on the Metro, eight hours at a monitor, a phone in your hand at every red light, then a laptop on the couch at night. By Thursday, there's a familiar burning knot between your shoulder blades and a neck that doesn't turn all the way when you check your blind spot.
I see this pattern in my clinic on 6th Street every week — office workers from the Wilshire corridor towers, nurses charting on computers, students, drivers, designers. The complaint is so common it has a name: tech neck. The name sounds trivial; the mechanics behind it are not.
The Biomechanics: What Your Head Actually Weighs at a Screen
Your head weighs roughly 10 to 12 pounds when it's balanced directly over your spine. That's the position your cervical spine evolved to carry — stacked, efficient, almost effortless.
The problem is that almost nobody holds that position at a screen. A widely cited biomechanical analysis by spine surgeon Dr. Kenneth Hansraj modeled what happens to cervical spine loading as the head tilts forward, and the numbers are worth sitting with (Hansraj, Surgical Technology International, 2014):
- 0 degrees (neutral): 10–12 lbs
- 15 degrees forward: 27 lbs
- 30 degrees forward: 40 lbs
- 45 degrees forward: 49 lbs
- 60 degrees forward (typical texting angle): 60 lbs
At the angle most of us hold while looking at a phone, the effective load on the neck is roughly equivalent to carrying an eight-year-old child on your cervical spine. Hansraj estimated that people spend two to four hours a day in this position on phones alone — 700 to 1,400 hours per year of abnormal spinal loading, before you even count the workday at a monitor.
What that load does over time
Your body adapts to whatever you do most. Hold a forward head position for years and a predictable pattern emerges, sometimes called upper crossed syndrome:
- The deep neck flexors — the small stabilizing muscles at the front of the neck — become weak and inhibited, because the forward-hanging head no longer asks them to work.
- The upper trapezius, levator scapulae, and suboccipital muscles become chronically overworked, because they're now holding a 40–60 pound effective load all day, every day.
- The chest tightens and the shoulders round forward, pulling the shoulder blades out of position and adding strain to the muscles that anchor them.
This is why I say tech neck "rewires" the neck and shoulders. It isn't just soreness; it's a learned postural pattern in which some muscles forget their job and others do double duty until they develop trigger points — those exquisitely tender knots that refer pain up into the skull and down between the shoulder blades. Tension-type headaches, jaw tightness, and that deep ache along the top of the shoulder are all downstream of the same mechanics.
Why Stretching Alone Often Fails
Almost every patient who comes to me with tech neck has already tried stretching. They stretch at their desk, they follow videos, some of them stretch diligently every day — and the relief lasts about twenty minutes.
There are three honest reasons for this:
First, the tight muscle is usually the victim, not the culprit. An upper trapezius that has spent eight hours holding up a forward-hanging head is tight because it is overworked and guarding, not because it's shortened. Stretching a muscle that's tight from overload can feel good briefly, but it doesn't change the load. The muscle re-tightens as soon as you sit back down, because the job hasn't changed.
Second, stretching doesn't reach trigger points well. Chronic overload creates contracted knots within the muscle that behave differently from general tightness. These respond far better to direct, targeted intervention — which is one reason needling techniques are effective where stretching is not.
Third, the nervous system is part of the pattern. Chronic muscle guarding is maintained centrally, not just locally. Stress, sleep debt, jaw clenching in traffic on the 110 — all of these raise baseline muscle tone through the nervous system. You can't stretch your way out of a sympathetic nervous system that's stuck in fifth gear.
None of this means stretching is useless. It means stretching alone is an incomplete answer to a problem that involves load, tissue changes, and the nervous system all at once.
The Korean Medicine View: Obstruction Along the Taiyang and Shaoyang Channels
Classical Korean medicine describes what modern biomechanics measures, using a different but remarkably consistent map.
The back of the neck and the region between the shoulder blades belong to the taiyang channels — the Small Intestine channel crossing the scapula and the side of the neck, and the Bladder channel running from the base of the skull down along the spine. The sides of the neck, the temples, and the jaw belong to the shaoyang channels — the Gallbladder and Triple Burner channels. If you trace where your tech neck hurts, you will almost certainly be tracing one or both of these pathways.
The classical principle is bu tong ze tong — where there is no free flow, there is pain. Hours of static posture do exactly what the classics warned about: prolonged sitting injures the flesh, qi and blood stop circulating freely through the channels that traverse the neck and shoulders, and stagnation sets in. Cold, aching, stiffness, and knots are all classical signs of qi and blood stagnation in the channel pathways. The biomedical description — sustained isometric contraction reducing local blood flow, leading to ischemic, tender tissue — is the same observation in different language.
Stress, the jaw, and the shaoyang connection
There's a second layer that purely mechanical explanations miss. In Korean medicine, the shaoyang channels are closely tied to stress physiology — the constrained, wound-up state that comes with deadlines, traffic, and the low-grade vigilance of modern work. When patients tell me their neck pain flares during a crunch at work, or that they wake with a sore jaw, I'm not surprised: jaw clenching, temple headaches, and tension along the side of the neck are a classic shaoyang presentation.
This is why two patients with "the same" tech neck may receive quite different treatments. One person's pattern is primarily taiyang — cold, stiff, postural, worse with sitting. Another's is shaoyang-dominant — stress-reactive, jaw-involved, flaring with emotional load. Recognizing the difference is where classical diagnosis earns its keep.
What the Research Says About Acupuncture for Chronic Neck Pain
I want to be straightforward here, because patients deserve more than enthusiasm — they deserve evidence. The research base for acupuncture in chronic neck pain is substantial and, on the whole, favorable:
- A 2016 Cochrane review of acupuncture for neck disorders found moderate-quality evidence that acupuncture relieves chronic neck pain better than sham acupuncture at short-term follow-up, with benefits for both pain and disability.
- The Acupuncture Trialists' Collaboration's individual patient data meta-analysis of roughly 20,000 patients — the most rigorous dataset in the field — concluded that acupuncture is superior to both sham and no-acupuncture controls for chronic musculoskeletal pain, and that treatment effects persist over time rather than fading quickly.
- A 2024 randomized controlled trial published in Annals of Internal Medicine found that individualized acupuncture produced clinically relevant, lasting improvements in chronic neck pain compared with usual care.
- A 2024 systematic review and meta-analysis focused specifically on durability found that the benefits of acupuncture for chronic neck pain are maintained well after the course of treatment ends.
Honest caveats: effect sizes versus sham are modest in some trials, study quality varies, and acupuncture is not a cure for a workstation that reloads your neck eight hours a day. What the evidence supports is what I see clinically — acupuncture meaningfully reduces pain and muscle guarding, and the relief tends to hold, which creates the window in which posture and strength work can actually succeed.
What Tech Neck Treatment Looks Like at Raah Acupuncture
At my clinic in Koreatown, treatment for tech neck typically has three components.
Diagnosis first. Before any needles, I assess your posture, neck range of motion, and the channel pathways involved — and I ask about what maintains the pattern: workstation, commute, stress, sleep, jaw. Pulse and palpation help me distinguish a taiyang-dominant pattern from a shaoyang one, and identify any underlying deficiency that keeps the tissue from recovering.
Saam acupuncture as the foundation. I practice Saam acupuncture, a classical Korean four-needle technique that treats the affected channels primarily through distal points on the forearms and lower legs. Rather than only needling where it hurts, Saam treatment adjusts the balance of the involved channels — say, draining excess along the Small Intestine channel or moving stagnation in the Gallbladder channel — so the whole pathway releases, not just one knot. Patients are often surprised that a needle near the elbow visibly softens the shoulder within minutes. That is the logic of the system working as intended.
Local and supportive techniques. Depending on your presentation, I combine this with targeted local needling of trigger points in the upper trapezius, levator scapulae, and suboccipitals, along with cupping or heat therapy to restore circulation to tissue that has been ischemic for months. Because stress and jaw tension so often drive the shaoyang component, treatment usually addresses the nervous system as well — most patients describe leaving in a noticeably calmer state, which is not a side effect. It's part of the mechanism.
A typical course for chronic tech neck is six to ten sessions, front-loaded at once or twice weekly, then spaced out. Most patients feel a meaningful change within the first three or four visits; if you don't, I'll tell you honestly and we'll reassess rather than repeating what isn't working.
The Honest Practical Section: What You Can Do Yourself
Acupuncture works best when you stop re-injuring the tissue between sessions. Here is what I actually ask of my patients.
Fix the geometry of your desk
- Raise your screen so the top third is at eye level. If you work on a laptop — at an office, or at a coffee shop on Wilshire — a riser plus an external keyboard is the single highest-value purchase you can make.
- Bring the screen closer rather than craning toward it. Your head should stay stacked over your shoulders.
- Raise your phone to eye level instead of dropping your gaze 60 degrees. This one habit removes hundreds of hours of abnormal load per year.
- In the car, adjust the headrest so it can actually support your head, and check your grip — many commuters carry their entire day's stress in their shoulders on the drive home.
Interrupt the load
The best posture is the next posture. Set a timer and stand, walk, or roll your shoulders every 30–45 minutes. Two useful movements: gentle chin tucks (sliding the head straight back, as if making a double chin) to reawaken the deep neck flexors, and a doorway chest stretch to counter the rounded-shoulder pull. Ten seconds each, many times a day, beats twenty minutes once.
Know when self-care isn't enough
Be honest with yourself about the trajectory. If your neck pain has lasted more than a few weeks despite improving your setup, if it's waking you at night, or if it now comes with headaches or jaw pain, the pattern has likely become self-sustaining and warrants treatment. And some symptoms should send you for medical evaluation promptly rather than to any conservative therapy first: pain radiating down the arm, numbness or tingling in the hands, arm weakness, or pain following an accident. I refer out when a case needs imaging or a physician's eyes — good care includes knowing the limits of your tools.

Is acupuncture better than massage for tech neck?
They work differently and can complement each other. Massage addresses broad muscle tension well, while acupuncture reaches deep trigger points precisely, treats the full channel pathway including distal points, and calms the nervous system component. In clinical trials, acupuncture's effects on chronic neck pain have been shown to last well beyond the treatment period, which is where massage-only relief often falls short.
Can acupuncture fix forward head posture?
Not by itself, and anyone who claims otherwise is overselling. Acupuncture reduces pain and releases the guarded, shortened muscles that make postural correction physically difficult — but reversing forward head posture also requires changing your workstation and rebuilding strength in the deep neck flexors. Treatment opens the window; your daily habits determine what walks through it.
